1. Purpose

The Permit to Work (PTW) system is designed to control high-risk activities by ensuring that necessary safety measures are in place before work commences. This system aims to prevent accidents, injuries, and incidents by managing work activities through a formal authorization process.

2. Scope

This policy applies to all employees, contractors, and third parties involved in high-risk activities at all LOCATION X RENT A CAR facilities.

3. Definitions

  • Permit to Work (PTW): A formal written system used to control certain types of work that are potentially hazardous.
  • Permit Issuer: A competent person authorized to issue permits.
  • Permit Receiver: The person responsible for carrying out the work as specified in the permit.

4. Types of Permits

  • Hot Work Permit: For work involving open flames or heat (e.g., welding, cutting).
  • Confined Space Entry Permit: For work in confined spaces (e.g., tanks, pits).
  • Electrical Work Permit: For work on or near electrical installations.
  • Excavation Permit: For any excavation work (e.g., digging, trenching).
  • Working at Heights Permit: For work conducted at height (e.g., scaffolding, ladders).

5. Permit to Work Process

5.1 Request for Permit

  • Initiation: The person responsible for the work initiates the permit request by filling out the PTW form.
  • Risk Assessment: A detailed risk assessment must be completed, identifying potential hazards and control measures.

5.2 Permit Issuance

  • Review: The Permit Issuer reviews the PTW form and risk assessment.
  • Authorization: If all safety measures are satisfactory, the Permit Issuer authorizes the permit.
  • Briefing: The Permit Receiver is briefed on the work scope, hazards, and control measures.

5.3 Permit Acceptance

  • Acknowledgment: The Permit Receiver acknowledges understanding of the permit conditions by signing the PTW form.
  • Pre-Work Inspection: A final inspection is conducted to ensure all safety measures are in place before work begins.

5.4 Permit Validity

  • Duration: The permit specifies the duration for which the work is authorized.
  • Revalidation: If the work extends beyond the initial period, the permit must be revalidated.

5.5 Permit Closure

  • Completion: Upon completion of the work, the Permit Receiver informs the Permit Issuer.
  • Inspection: A post-work inspection is conducted to ensure the area is safe.
  • Closure: The permit is closed, and all documentation is filed for record-keeping.

6. Responsibilities

6.1 Management

  • Ensure the PTW system is implemented and followed.
  • Provide necessary training and resources for the PTW system.
  • Regularly review and update the PTW system.

6.2 Permit Issuers

  • Authorize permits after thorough review.
  • Conduct briefings and inspections.
  • Maintain records of all permits issued.

6.3 Permit Receivers

  • Comply with all permit conditions.
  • Ensure the work is carried out safely.
  • Report any incidents or hazards encountered during the work.

7. Training

All employees and contractors involved in high-risk activities must undergo training on the PTW system, including:

  • Understanding different types of permits.
  • Completing and reviewing PTW forms.
  • Identifying and controlling hazards.
  • Procedures for permit issuance, acceptance, and closure.

8. Monitoring and Compliance

  • Audits: Regular audits of the PTW system to ensure compliance and effectiveness.
  • Incident Investigation: Investigation of any incidents related to permitted work to identify root causes and preventive measures.
  • Continuous Improvement: Regular feedback and updates to improve the PTW system.

9. Review

This policy will be reviewed annually and updated as necessary to ensure its continued relevance and effectiveness.

This PTW system document aims to manage high-risk activities effectively, ensuring a safe and secure working environment at LOCATION X RENT A CAR.

Any non-routine job that requires special safety precautions and/or inter-departmental communications is subject to the Work Permit system. Driving into a permit-to-work environment will certainly require a permit-to-work order for a car rental situation.
